The second document you need is evidence of change of name. This is to show that you have adopted your husband’s surname after marriage. You can use a newspaper publication of your change of name as evidence.
You can also use a sworn affidavit from a court of law or a gazette notification from the government. Make sure that your evidence of change of name has your old name, your new name, and the date of publication or issuance.
The third document you need is your husband’s place of residence. This is to show that you are living with your husband in the same state or that you have his consent to serve in a different state.
You can use a utility bill, such as electricity, water, or telephone bill, as proof of residence. Make sure that the utility bill has your husband’s name, his address, and the date of payment.
The fourth document you need is your husband’s ID card. This is to show that you are married to a Nigerian citizen and that he is eligible to vote or pay taxes in the country.
You can use any of the following ID cards as proof: driver’s license, national ID card, voter’s card, international passport, or permanent voter’s card. Make sure that the ID card has your husband’s name, his photograph, and his signature.
The fifth document you need is a letter of consent from your husband. This is to show that he agrees with your decision to join the NYSC and that he supports you throughout the service year.
The letter of consent should be handwritten by your husband and addressed to the NYSC director-general. It should state your full name, your husband’s full name, your state of origin, your state of deployment, and your reasons for joining the NYSC. It should also have your husband’s signature and date.
These are the five documents you need to register for NYSC camp as a married woman. Please note that you should upload copies of these documents on the NYSC portal during online registration.
